Client Intake ~
Welcome! I am so pleased to be of service to you. Let’s build a good partnership. You will be prompted to complete an intake form when you set your appointment. Please provide complete information so we can get to work as soon as you arrive.
Your personal information is private. I will not use or disclose information unless I am required to do so by law or as authorized by you. I will not use or sell protected health information for marketing purposes.
Length of sessions: Please return the completed form at least the day before your appointment to receive treatment for the entire allotted time.
Late arrivals: If you arrive late, you will still receive treatment within the originally allotted time. (For example: You arrive 15 minutes late to a 1-hour session. You will receive treatment for 45 minutes.) If you will be more than 30 minutes late, please reschedule.
Cancellations: If you fail to give 24 hours’ notice, you will be charged 50% of the session fee.
